Individual Membership
Patients who want better access and coordination without a full concierge relationship.

- Physician-directed referrals and care coordination
- Prescription refills and medication management
- Preferred lab pricing — $150 per panel
- Preferred visit pricing — $250 per visit
- Super bill provided for private insurance
Tier 1 is visit-based care.
Concierge Care
Individual or Couple
Individuals and families who want responsive, relationship-based medical care.
- Same-day or next-day appointments when clinically appropriate
- Extended, unhurried visits
- Two comprehensive lab panels per adult annually
- Ongoing review of labs and health trends
- Physician-led care coordination and specialist referrals
- Imaging ordered when clinically appropriate (paid by patient)
Children 5–17 included (up to 5) · Young adults 18–26: $125/mo
Tier 2 is high-touch, responsive concierge care.
Executive Concierge
Individual or Couple
Patients who want proactive, physician-led preventive planning and the highest level of clinical oversight.
- Everything in Tier 2, plus:
- Monthly preventive risk assessment
- Proactive long-term health strategy
- Physician-guided advanced diagnostics consideration
- Priority coordination for advanced imaging and specialty diagnostics
- Enhanced physician oversight in long-term health decisions
- Full body MRI preventative cancer screening included
Children 5–17 included (up to 5) · Young adults 18–26: $125/mo
Tier 3 is defined by prevention and strategy — not more tests.
